Case Management:


Seniors (and family members) are supplied with a better understanding of COA services, are referred for other services (COA programs as well as other service organization programs) for which they are eligible, and/or are assisted in finding the resources to meet their needs.  Case managers assist with EPIC applications and are also able to answer questions regarding Medicaid and Medicare.  Case managers are available to identify needs, develop and monitor service plans to assist in meeting the identified needs, and to advocate for and communicate with other agencies when necessary for additional services.
